Where it is being replaced, CMS's crosswalk moves members into the plan or plans shown in the table. Your Annual Notice of Change explains the new plan. In some of these counties, CMS's file also lists the plan as ending, so some members may not be moved. They need to pick a plan.

If this plan is not offered in your county in 2027, you can pick a new plan from October 15 to December 7, 2026. If your plan is ending, you also have until February 28, 2027. That extra time doesn't apply if your plan is being combined into another plan. Check your county to see what happens if you do nothing.
