In some counties, CMS's crosswalk moves members of this plan to more than one plan. CMS's file doesn't say which members go where. Your Annual Notice of Change says which plan is yours.

If this plan is not offered in your county in 2027, you can pick a new plan from October 15 to December 7, 2026. If your plan is ending, you also have until February 28, 2027. That extra time doesn't apply if your plan is being combined into another plan. Check your county to see what happens if you do nothing.
